Administrator: MedBenThe University offers employees a choice between one of two medical insurance plan options or a high deductible health insurance plan with a Health Savings Account ("HSA"). All plan options include a retail and mail order prescription program.
Please review the Plan Summary and Premium Comparison Chart to see which option is right for you.
MedBen uses the Ohio PPO Connect network of providers for regional care. If you need emergency care during temporary, out-of-state travel, use your MedBen insurance card. Should you spend a substantial amount of time out-of-state each year, please contact the Human Resources Office to discuss whether coverage under the nationwide PHCS network is appropriate for you.
Employees who wish to enroll in Muskingum's health insurance program must do so during the times indicated below. Plan selections may not be modified except in the event of a qualifying life event or during open enrollment each December. Employees must complete the enrollment form to enroll in new coverage or to modify existing coverage and submit the form to the Human Resources Office.
Health Savings Account
As noted above, Muskingum offers a self-funded HSA option to employees. Employees who participate in the designated high deductible health care plan and contribute to an HSA may not participate in the University’s medical Flexible Spending Account; however, unlike the FSA, the HSA funds are not forfeited at the end of the plan year. Employees who are 65 or older may not contribute to an HSA based on IRS regulation. For employees who do contribute to the HSA, contributions are in addition to the premium for the high deductible health plan. The money accrued in an HSA is available to pay for eligible medical expenses and will remain with the employee throughout their employment and after they separate from employment. Employees should review their personal medical needs, as well as financial and tax needs, when determining whether an HSA or one of the two traditional PPO plans is the best option for them.

-
Where medical, dental, and/or vision coverage ends, employees and their families have the option to purchase a temporary extension of their insurance plan at group rates. This is sometimes referred to as “continuation coverage.” The reasons why continuation coverage may be available include reduction of hours or separation from service, unless due to the employee’s gross misconduct. Dependents may also purchase continuation coverage if the employee's insurance is discontinued due to divorce, the employee’s death, or the dependent reaching a disqualifying age.
MedBen manages the COBRA programs for both health and dental benefits. MedBen will provide enrollment information to the individual and collect and process payment for continuing coverage.
The Human Resources Office manages the COBRA program for vision insurance. The Human Resources Office will provide enrollment information and the collection and processing of payment for continuing coverage.
