• Project request (PDF) - Use for scheduling and consideration of research and development initiatives.
• Direct link to Automated Blackboard course request (Web form) - Use to request creation of a Blackboard course. The course must exist and you must already be the instructor of record in the Registrar's database.
• New acquisition Technology request (PDF) - Use to seek approval to purchase new equipment.
• Asset Relocation/Reassignment (Web form) - Use this form to either notify us of technology changes you are making or to request our assistance in your asset relocation and/or reassignment.
• De-Commissioned computer request (Web form) - Use to request distribution of a 5 year or older computer.
• "Green" Computer disposal (PDF) - Use to transfer ownership of a computer to an off campus owner. (Becomes the responsibililty of the new owner.)
• Network Connection (PDF) - Use to request network/internet access for a computer acquired through independent means which is not in the yearly rotation process or centrally budgeted.
• Administrative Data Access (PDF) - Use to request access to the Datatel database. (To be used by departmental managers upon staff addition or change). Read the Administrative Data Access web page which describes the process.
