Registration & Enrollment Forms

The following forms are required by ODJFS and MUCCD. All forms must be completed before the child's first day of attendance unless otherwise indicated and annually thereafter.*

Pre-school Enrollment Forms

• Complete Preschool Enrollment Forms 2013-2014 (20 pages)

Individual Forms as needed:

• Preschool Enrollment Form Ages 3-5
• Child Enrollment and Health Information for Child Care Centers
• Child Medical Statement *Required within the first 30 days of enrollment
• Child Dental Screening *Required within the first 30 days of enrollment
• Permission to Release
• Media Release Form
• Request for Administration of Medication (Sunscreen) *On an as needed basis
• Childcare Plan for Health Conditions or Medical Procedures *On an as needed basis
• Routine Field Trip Permission
• Permission to Participate in Swimming Activities
• Release and Indemnification Agreement
• Initial Parent Survey*Upon initial enrollment only
• Preschool Supply List

School Age Enrollment Forms

• Complete School Age Registration Forms 2013-2014 (17 pages)
