Use this form to request utilization of a computer/computers formerly on the 4 year replacement program. Systems become available after the primary replacement program has been completed (i.e. after Fall classes have started).

Upon submission, your request will be reviewed and considered by the VPAA prior to any determination. If you are granted utilization of a de-commissioned computer(s), you will also be asked to sign the Computer Disposal Form, accepting responsibility for its use and/or ultimate disposition.
